This four-part course series is designed to be taken sequentially, beginning with Course 1: Behind the Scenes With SLPs Across Models of Care in the ICU and concluding with Course 4: Swallowing Treatment: What’s Next?
Each course builds upon the knowledge from the previous one, providing learners with an understanding of ICU management for SLPs. By the final course, participants will have a well-rounded grasp of key concepts essential for effective patient care in the ICU.

Course Overview Patient care for dysphagia requires a diagnostic workup. Validation, reliability, and sensitivity of the tools used matter, regardless of approach. Understanding the potential threats to normal swallowing, voice, and cognitive function should drive the diagnostic process. Often, there are intricate relationships between patients’ comorbidities. Understanding the patient’s history and current medical approach/plan from the medical record and discussions with the multidisciplinary team will lead to better choices in diagnostic tools. With tools in hand, an organized and pragmatic approach to assessment may be completed to create an individualized, patient-centered management plan. This course will discuss the necessary components of screening and diagnostics for swallowing disorders/dysphagia. It will identify potential threats to normal swallowing, voice, and cognitive function in critically ill patients to highlight appropriate goals in the treatment plan.

Learning Objectives
  • Evaluate ICU models and their resources and how they impact the referral process
  • Assess validated screening tools to identify potential threats to swallowing, voice, and cognitive functions
  • Develop a plan for effective medical chart review
  • Formulate approaches for an efficient and accurate swallowing assessment leading to a therapeutic plan in alignment with the patient and the multidisciplinary team’s goals of care

Chapters & learning objectives

Models of SLP Services in the ICU

1. Models of SLP Services in the ICU

Participants will examine referral patterns and current resources to drive referrals for swallowing assessment in the ICU. Priorities for referrals will be identified to assist the multidisciplinary team in identifying necessary and appropriate candidates.

Screening Dysphagia in the ICU

2. Screening Dysphagia in the ICU

There are no losers in screening tools. The question is which tool will accomplish the intended goal(s) for the multidisciplinary team. Participants will examine who is appropriate for screening, who is trained to screen, and at which time.

Efficient and Purposeful Medical Chart Review

3. Efficient and Purposeful Medical Chart Review

A comprehensive review of the medical record provides the necessary information about the impact of the patient’s diagnosis on swallowing function. The PARTNERS framework will help guide the medical history interview to identify patient-specific goals for an optimal treatment plan.

Aligning Approaches to Swallowing Assessment With Goals of Care

4. Aligning Approaches to Swallowing Assessment With Goals of Care

Each clinician knows the tools that are available within each institution for swallowing assessment. Application of these tools within the ICU requires complete understanding of the multidisciplinary team’s and patient’s/caregivers’ goals within the framework of PARTNERS. Aligning goals requires skills to synthesize information and develop a patient-centered approach to assessment.
